A year ago, the biggest AI security concern was employees pasting sensitive data into ChatGPT. That's still a problem, but 2026 has brought new risks that most security training hasn't caught up with.

Here are the five mistakes we're seeing most often in our security assessments.

1. Trusting AI-Generated Code Without Review

Your developers are using Copilot, Claude, Cursor, or similar tools. Good. They're also shipping AI-generated code straight to production. Bad.

The problem isn't that AI writes insecure code (though it sometimes does). The problem is that developers trust it more than they should. When code appears "complete," there's a psychological tendency to skip the security review.

The fix: Treat AI-generated code like code from a new junior developer. It might be brilliant, it might be terrible, but it always needs review. Update your code review checklist to include "AI-assisted" as a flag that triggers extra scrutiny.

2. Using Free Tiers for Sensitive Work

Enterprise AI plans exist for a reason. When your team uses free ChatGPT or free Claude to work on client data, that data may be used for training. The terms of service for free tiers are different from paid tiers.

We've seen companies with strict data policies whose employees use personal accounts to "get around the IT restrictions." They think they're being resourceful. They're actually creating liability.

The fix: If AI is useful for work, pay for it. Make the enterprise version easier to access than the workaround. Block personal AI tool usage on work devices if necessary.

3. Ignoring Context Window Persistence

Many employees don't realize that AI assistants remember everything in the conversation. They'll discuss Project A, then switch to discussing Project B in the same chat. Now the AI has context about both, and if it hallucinates or gets confused, it might reference the wrong project.

Worse: some tools now have persistent memory across sessions. Information shared in January might influence responses in March.

The fix: Train employees to start fresh conversations for different projects. Use tools that let you disable persistent memory for sensitive work. Periodically audit what your AI tools are "remembering."

4. Over-Relying on AI for Compliance Decisions

"Is this HIPAA compliant?" "Does this meet GDPR requirements?"

AI can help you understand compliance frameworks. It cannot give you legal advice. But we're seeing teams treat AI-generated compliance checklists as authoritative, without verification from legal or compliance professionals.

AI models are trained on historical data. Regulations change. Interpretations evolve. What was compliant when the model was trained might not be compliant today.

The fix: Use AI as a research assistant, not a compliance officer. Always verify AI-generated compliance guidance against current regulations and with qualified professionals.

5. Not Monitoring AI Agent Actions

This is the 2026 problem that barely existed in 2024. AI agents can now browse the web, execute code, send emails, and interact with your systems. When something goes wrong, do you know what the agent did?

Many teams have given AI agents broad permissions without implementing proper logging. If an agent makes a mistake or is exploited, there's no audit trail.

The fix: Log everything your AI agents do. Implement approval workflows for high-risk actions. Regularly review agent activity. Treat AI agents like you'd treat a contractor with access to your systems.

The Common Thread

All five of these mistakes stem from the same root cause: treating AI tools as magic rather than as tools.

Tools require training. Tools require policies. Tools require oversight.

The companies that will thrive with AI are the ones that build these foundations now, not the ones that move fast and clean up later.
